Style #1:  Single line melody (1LM)
This is our most common type of lead sheet.  It is simply a single-staff, one-note melody line with lyrics below, and chords above.

Style #2:  Single line 3-part harmony (1L3)
We have found this useful in church settings where there is a band and a choir.  It is a single-staff, 3 part harmony (Soprano, Alto, Tenor) with lyrics below, and chords above
Style #3:  Two-line 4-part harmony (2L4)
This is great for choirs.  It is 2 staves (Treble & bass) with 4-part harmony (Soprano, Alto, Tenor, and Baritone/Bass). Lyrics are between the two staves, and chords at top.

Style #4:  Lead with Piano (PIA)
This style is simply a 1-line lead with piano accompaniment.  Please note:  the cost for a Lead with Piano is $10 extra per song.
